தேசிய அடையாள அட்டையைப் பெறுவதற்கு

1. தகுதிகள்

• இலங்கையின் குடிமகனாக இருத்தல்
• 15 வயது இருக்கும்

2. விண்ணப்பங்களை பெறலாம்

• விண்ணப்பத்தைப் பதிவிறக்கவும் - National Identity Cards (Pre-Processing) or
• அனைத்து மாவட்ட செயலாளர்கள்
• அனைத்து பிரதேச செயலாளர்களும்
• அனைத்து சான்றளிக்கும் அலுவலர்களிடமிருந்து (கிராம அலுவலர், அதிபர், தோட்ட கண்காணிப்பாளர்)
• துறை இணையதளத்தில் இருந்து (www.drp.gov.lk)
• பயன்பாட்டின் தெளிவான மற்றும் ஒத்த அளவிலான நகல்.

3. Documents to be forwarded

1. The originals of the following documents certified by the Additional District Registrar, together with 1.7.8 form of the Department for Registration of Persons (The documents provided should confirm the Name and the Date Of Birth of the Applicant)

• Birth Certificate or an extract obtained from the Births Register.
• A document to confirm the date of birth ( Birth Certificate of a Child/ School Leaving Certificate/Estate Leaving Certificate/ A copy of an acceptable Horoscope/Marriage Certificate/ Citizenship Certificate/ Original of the Affidavit) together with Probable Age Certificate.
• Citizenship Certificate/ Dual Citizenship Certificate

2. Birth Certificates of those who are above 40 years and the following documents to confirm the age of those who do not own a probable age certificate.

• The result sheet of document search issued by the Additional District Registrar.
• An acceptable written proof to confirm the name and the date of birth.( A birth Certificate of a Child/School leaving certificate, Estate leaving Certificate/ Certified copy of the Horoscope/ Marriage Ceritificate/ Citizenship Certificate/ Adult Identity Card)
• First copy of a formal affidavit to confirm the name, date of birth and place of birth.

3. The first copy of the Service Confirmation latter in order to confirm the profession.
4. Three color photos of 13/8«7/8 inches each.( The background color must be sky blue)

The photos must be obtained from a studio registered at the Department of Registration of Persons.

5. Other documents required when issuing Identity Cards.

• Samanera Certificate or the Upasampada Certificate issued by the Department of Buddhist Affairs
• Certificates that confirm clergy, for clergy of other religions.(issued by the relevant Department of Religious affairs.
• If the relevant clergy has left monkhood/ clergy, certificate issued by the relevant Department confirming that.
• Marriage certificate to obtain the name of the husband.
• If a person born abroad to a Sri Lankan mother and a father, Citizenship Certificate issued by the Department of Immigration and Emigration to confirm that Citizenship of Sri Lanka is obtained.
• Dual Citizenship Certificate if the applicant is a Person with Dual Citizenship.
